Description
This session reframes case management as more than coordinating tasks, referrals and plans. Through a narrative-practice lens, participants will consider how assessment conversations can make room for context, identity, culture, strengths and preferred futures. The session will explore collaborative formulation, shared language, documentation and the balance between organisational requirements and person-led practice.
Who it is for
Community and human services practitioners involved in assessment, case planning, coordination or ongoing support, across all levels of experience.
